Welcome to Continuing Care Coordinators
Continuing Care Coordinators (CCC) is a professional organization established to address the current issues surrounding continuing care. We are health care and social services professionals who provide direct or inter-related health services with the Milwaukee area and neighboring areas throughout Southeastern Wisconsin.
We have four general meetings and two special events per year.
