Who Are We?
Continuing Care Coordinators (CCC) is a professional organization established to address the current issues surrounding continuing care. We are health care and social services professionals who provide direct or inter-related health services with the Milwaukee area and neighboring areas throughout Southeastern Wisconsin.
CCC began in 1971 with hospital home health care coordinators, representatives of health departments and non-profit home health providers.
Membership has grown over the years to now include hospital social workers and discharge planners, assisted living facilities, nursing homes, subacute care centers, social service agencies, public health professionals, home care providers, home medical equipment suppliers, insurance providers, HMO’s, managed care organizations, and other community health care professionals.
Our purpose:
To promote the continuity of care
To foster high standards of health care practice
To support the integration of community programs and resources
To promote professional development, educational resouces and peer networking
To promote advocacy programs with local government agencies
To increase awareness and participate in the legislative issues
To provide Continuing Education Units (CEUs)
CCC membership is open to anyone interested in Continuing Care. Membership dues for an individual are $40 for the year and $100 for a corporate membership. Membership rate includes all meeting fees, includes a continental breakfast at all General Meetings and free attendance to our Annual Educational Symposium held in May. CCC has six events per year, four General Meetings and two Special Events. The General Meetings are held on the third Wednesday of the month in January, March, July and September. Meetings are from 7:30 a.m. to 9:30 a.m. Guests are welcome to attend General Meetings for a guest fee of $10.
In addition, CCC has two special events each year. In May, we hold our 1/2 day Annual Educational Symposium. In November, we have our Annual Networking Fair.
